What we want The Registered Nurse - Diabetes Education Program, is responsible for providing diabetes education/care instructions and performing initial/re-assessments for program participants. This role acts in accordance with Professional Nursing Standards. What you need to know Other Requirements and Factors: Ability to work beyond normal office hours and/or on weekends as needed. Ability to travel to various locations. Required Education and Experience: * Successful completion of a registered nurse education program. Required Licenses and Certifications: * Licensed in the State of Texas as a Registered Nurse. CPR certification. * American Association of Diabetes Educators (AADE) Diabetes Paraprofessional Level 2 certification, or ability to obtain certification within 3 months of employment. * Valid Texas driver's license or ability to obtain one within 30 days of employment. * Licensure/certifications must be maintained for continued employment. Required Knowledge, Skills, and Abilities: * Ability to multitask and work cooperatively with others. * Ability to work with sensitive information and maintain confidentiality. * Ability to use and manage an electronic medical records (EMR) system. * Strong written and verbal communication skills. Knowledge of standard nursing procedures and techniques. * Knowledge of word processing and spreadsheet applications. * Knowledge of confidentiality laws and regulations, including HIPAA and FERPA. * Interpersonal and organizational skills. Preferred Qualifications: * Certified Diabetes Educator. * Bilingual (English and Spanish). Responsibilities: Education * Provides diabetes self-management education/support. * Develops individualized education and support plans and monitors progress on a quarterly basis. Collaborates with referring providers/other healthcare professionals who are outside of the Diabetes Self-Management Education and Support (DSMES) service regarding education provided, education plans, and outcomes. * Provides preventive education to the community and students. * Assumes responsibility for assigned education locations, includes travel to assigned locations. Participates in continuing education activities to maintain licensure/certification(s). Assessments * Performs comprehensive initial assessments and reassessments for class participants' education and health care needs, including diabetes self-management knowledge, skills and readiness for behavior change. * Collects biometric data, including blood pressure, heart rate, weight, BMI, body fat percentage, blood glucose, and hemoglobin A1c (HBA1c). * Maintains and operates biometric equipment for initial and quarterly reassessments. Documentation * Documents and maintains class participants' education records and health care needs. * Maintains State Department of Health records as required. House Bill 984 Training * Provides training to school personnel who are not licensed healthcare professionals in the care of elementary and secondary school students with diabetes. Inventory * Maintains inventory of medical supplies and equipment. * Communicates supply/equipment needs to the appropriate individual. * Performs other duties as assigned. * May provide supervision for support staff as needed.
