Manages all outpatient advanced heart failure patient issues under the supervision of the heart failure cardiologists. Participates in all outpatient visits. Utilizes critical thinking to interpret clinical symptoms, abnormal laboratory values, pulmonary artery waveforms in the setting of CardioMems devices, and manages intravenous inotropic medications. Works in unison with the OHT/VAD team members to assure smooth transition for patients across the health care continuum. Must be able to effectively communicate with patients, families and all other members of the patient care team. ESSENTIAL FUNCTIONS: Collaborates with physicians and assists in the coordination and follow through of the cardiac clinical plans documented. Triages patient calls which require a high degree of clinical judgement or which may require the need for prompt assessment and intervention. Manages all inotrope dependent patients. Acts as a liason for home infusion nurses and pharmacists. Makes medication adjustments based on weekly lab results. Assesses and evaluates patients in the outpatient clinic with physicians and ensures documentation is updated and accurate in EPIC. Manages all anticoagulation for heart failure patients and oversees bridging for these patients with regard to planned surgeries and procedures. Provides education to patients and families regarding heart failure disease process and management, medication and diet. Inpatient rounding with AHF physician to help with patient education, transition of care and discharge planning. OTHER FUNCTIONS AND COMPETENCIES needed in addition to Essential Functions: Reviews and analyzes all incoming lab results and addresses all abnormal results. Assures the completion of all necessary forms including disability, homecare, hospice, FMLA and medical clearance. Facilitates the direct admission of patients when necessary both planned and emergent. Participates in weekly candidacy meetings for potential heart transplant and VAD patients. Manages all Cardiomems patients with regard to diuretic titration and appropriate lab follow up. Requires the ability to interpret pulmonary artery pressure waveforms. Facilitat s genetic testing for appropriate cardiomyopathy patients. Collects specimens and assures they are sent promptly to the company for processing. Screens all daily MCOT reports for arrythmias and reports urgent issues to physicians requiring understanding of basic cardiac rhythms. Enrolls patients in cardiac rehab programs. Facilitates the scheduling of outpatient procedures including all appropriate pre-procedure labs and instructions. Participates in the ongoing screening of potential research candidates. Manages data and communicates with providers and clinical subjects accordingly. Arranges for intravenous iron infusions and intravenous diuretic administration through the JHN infusion center. Completes a large volume of medication prior authorizations for heart failure and amyloidosis medications. Travels to satellite clinics to assist providers both in Philadelphia and New Jersey. Oversees transitions of care from the inpatient to the outpatient setting with the participation of the multidisciplinary team. Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. EDUCATIONAL/TRAINING REQUIREMENTS: Graduate of an accredited school of nursing required; BSN or completion of BSN within 3 years of hire strongly preferredCERTIFICATES, LICENSES, AND REGISTRATION: Licensed Registered Nurse in the Commonwealth of PA required. Licensure in the State of NJ preferred ACLS certification CITI Certification within 6 months of hire CHFN or CHF-N certification through AAHFN preferredEXPERIENCE REQUIREMENTS: 3+ years of experience with Heart Failure patients strongly preferred. Background working with a cardiac physician practice, a cardiovascular lab or at least 2 years hospital nursing experience is highly desirable. Experience working with critical care services strongly preferred ADDITIONAL INFORMATION: Familiarity with basic cardiac rhythms and knowledge of swan ganz waveform interpretation helpful.
