Our integrated system emphasizes prevention, healthy living and is designed to reduce health care disparities and avoid unnecessary trips to emergency rooms or other more costly forms of care.

Desert Sage Health Centers is currently recruiting an energetic full time Referral Coordinator team member who is self motivated, energetic, and approaches customer service with a smile first, for our clinical department! This individual collaborates with other health center team members assisting in the promotion of the health and well-being of patients. If this sounds like a good fit, then please apply!

Responsibilities:

  • Assist with tracking, monitoring, and follow-up on referrals to internal and external providers.
  • Maintain all referral coordinator buckets.
  • Process urgent in-house Medical Records Requests for patients.
  • Complete all prior authorizations initiated by referrals requested by the patient's Primary Care Provider.
  • Work with the guidance of the care team to contact patients post-discharge and schedule follow-up care.
  • Maintain strict confidentiality on all patient information.
  • Assist patients in the completion of the 340B and Medication Assistance Programs.
  • Perform all other duties as assigned.

Qualifications:

  • 3 - 5 years of customer service/healthcare experience preferred.
  • Bicultural/bilingual Spanish fluency preferred.
  • Experience working with Hispanic populations preferred.

Education: 

  • High School Diploma Required
