The Home Care RN is a professional caregiver who coordinates and provides direct and indirect patient care using the nursing process to meet the physical, psychosocial, environmental, and spiritual needs of specific patient populations and families within a designated area.

  • Understanding of Medicare Home Care benefits and regulatory requirements
  • Case management activities including assessment, planning, monitoring, and evaluation
  • Completion of required documentation and timely communication with healthcare team members
  • Practicing in a sensitive manner to the needs of patients and families, and directing the activities of licensed practical nurses and home health aides
  • Performing OASIS assessments, complex wound care, and central line monitoring
  • Communication with physicians regarding patient updates and condition changes
