*Job Title: * Psychiatric Nurse Practitioner

*Schedule: 2-3x week (in-person)*

*Work Environment: *The Teen Project, Inc. operates the Freehab Crisis Residential Program, a state-licensed residential facility for up to 16 female adults*.* The Crisis Residential Program is a 24-hour facility that provides round-the-clock treatment and care for its residents in a caring and compassionate environment. This is an emergency housing facility for residents being released from (but not limited to) acute, short-term hospital care. The program’s goal is to obtain permanent and stable residential placement within 30 days of admission.

Under the supervision of the consulting psychiatrist, the family nurse practitioner will be on site three to four days per week and on-call as needed for consultation. The family nurse practitioner will provide medication assessment/support services, including administration of prescribed medication in an emergency, basic physical healthcare and education both individually and in a group setting, and staff training. The family nurse practitioner promotes the mission of The Teen Project, Inc. by providing culturally comprehensive assessment and health services, participates in treatment planning and provides patient education within their scope of practice as designated by The Teen Project, Inc. as well as their licensing or registering body.

The psychiatric nurse practitioner is an energetic self-starter who is able to function comfortably in a team environment or independently and relates well to co-workers and County representatives. This position requires flexibility and the ability to manage time and multiple tasks with little supervision. The individual in this position must be able to assume a wide range of responsibilities, provide timely responses to requests and deadlines and work well under pressure, as well as provides crisis intervention and other direct services.

*Essential Job Functions: *
* Provide medication assessment/support services
* Track and manage prescriptions, refills and contact pharmacy as needed.
* Diagnose and treat medical diagnoses.
* Consult with referring hospital medical staff.
* Consult with psychiatrist.
* Administration of prescribed medication in an emergency.
* Provide crisis stabilization to clients as needed.
* Provide basic physical healthcare and education to clients.
* Coordinate medication access and instructions upon client discharge
* Conduct staff trainings.
* Provide services in the office, consistent with contractual, programmatic and funding source requirements.
* Assess and monitor client progress; evaluate outcomes and collaborate to adjust the course of treatment as appropriate.
* Complete service and coordination plans, progress notes, and other County-required documentation within the time frames as established by the Agency; meet the timeliness standards of the state and county government funding and other agency funding sources.
* Meet agency requirements regarding client service hours, both billable (for reimbursable services) and-billable; consistently provide services in keeping with all programmatic and contractual requirements.
* Interact well with internal programs, clinical/support staff and external sources such as school or community personnel as applicable.
* Attend and actively participate in staff meetings to provide input towards program development and staff training.
* Manage all information containing sensitive/confidential content with utmost discretion and adherence to all relevant policies and procedures. Maintain confidentiality for all sensitive matters.
* On-call as needed.
* Provide program coverage and assistance for other departments as needed or as assigned.
* Via consultation with relevant supervisors, meet clinical quality expectations.
* All other relevant duties as assigned.

*Minimum Qualifications: *
* Graduate education at the masters, post-master’s or doctoral level and national board certification as Nurse Practitioner
* Certification for Psychiatric Mental Health practice and ANCC Certification of Psychiatric Mental Health program from an accredited university
* Registered with

* Bilingual ability highly preferred (i.e., Spanish or other languages relevant to the clients’ needs).
* DMH paperwork experience preferred.
* One year experience in residential mental health setting
* Requires the ability to independently use a computer and learn necessary computer programs.
* Must possess a valid California driver's license and maintain an insurable driving record under the agency’s liability policy. May be required to obtain a Class II California driver’s license.
* Must pass all applicable health, fingerprint/Live Scan, and DMV clearances.
