Advanced Practice Provider - In-Home Care

We are seeking Advanced Practice Providers with acute care experience in ER, Urgent Care, or Internal Medicine to join our team providing in-home care. Our providers will work as part of a clinical care team to deliver comprehensive health assessments, diagnosis, treatment, and outcome management for patients of all ages. Responsibilities include performing therapeutic interventions, analyzing test data, educating patients and families, and effectively communicating with all individuals involved in the patient's care. The ideal candidate will have a minimum of two years of experience as an APP in relevant areas, hold an advanced degree from an accredited institution, and possess current unrestricted state license as a Nurse Practitioner or Physician Assistant.
