Works under the direction and supervision of a registered nurse. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge. Qualifications Qualifications: * High School Diploma preferred. * Must have current driver's license. * At least one year experience as a Home Health Aide or Nursing Assistant in a hospital, nursing home or home health agency, or completion of Certified Nursing Assistant or Certified Home Health Aide Program. * Good communication skills to be able to relate professionally and positively to patients, caregivers, and clinical team. * Ability to read, write, and speak the English language * Demonstrates interest in the welfare of ill and elderly. * Proof of current, CPR and Hepatitis profile. * Aide Competency Evaluation Skills Checklist and written Skills Test. Essential functions: * Provide direct patient care to patient under direction of the RN and according to the Aide Plan of Care. * Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy. * Provide necessary skill to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care. * Practice accepted infection control principles. Provide a clean, safe and comfortable environment. * Promote positive, supporting, respectful communication to patient/family and other employees. * Provide an environment which promotes respect for patient, privacy and property. * Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake. * Contribute to the management and efficient operation of the agency and demonstrate effective time management skills. * Provide skills necessary to perform treatments and procedures according to agency policy. * Demonstrate commitment, professional growth and competency by attending 12 hours of in-services per year. * Promote the agency philosophy and administrative policies to ensure quality of care. * Records clinical documentation in medical record. * Participates in weekly case conferences. * Participates in interdisciplinary communication to ensure that patients' needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress. * Responsible for assuring patient safety. * All documentation will be completed and synched by 9:00AM the following day, 7 days per week.
