Responsibilities:
- Provide professional nursing services under the direction of a RN in compliance with healthcare standards and regulations.
- Deliver skilled nursing interventions, rehabilitative care, and preventative treatments in accordance with the plan of care.
- Confer with the RN Supervisor regarding needed changes in the Plan of Treatment and communicate physician orders.
- Maintain contact with patients, clinical managers, and other healthcare team members to ensure coordination of care and discharge planning.
Requirements:
- Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
- Current nursing license in the practicing state and CPR certification.
- Two years experience as an LVN in a clinical setting, preferably in home health or hospice.
Hourly Commitment:
Full Time (40 hours per week)
Compensation:
$48,600 - $67,000 per year
