The LPN is a member of a health care team, which includes, a provider, a behavioral health consultant, a registered nurse (RN), a medical assistant, a care coordinator, and a patient service representative (PSR) that have been assigned a panel of patients. Working within the full scope of LPN licensure, the LPN provides direct nursing care under the immediate supervision of a registered nurse, physician, advanced practice nurse and/or physician assistant. Scope of duties are to include, but not limited to obtaining and recording vital signs, discussing chief health concerns, performing, document and follow up on laboratory tests, assisting with specialized procedures, perform a variety of tests (vision, peak flow, lung function, EKG's, etc.), obtain samples for lab tests through venipuncture, nasal swabbing, etc and administer and chart medications, and perform injections, including immunizations. Must maintain education requirements to sustain certification and attend continued education as required by supervisor, state or federal guidelines. Provide LPN coverage at other locations as needed.


Minimum Qualifications:
• Possess licensed practical nurse license.
• Possess current CPR certification
• Produce documentation of appropriate phlebotomy skills.
• Must have the ability and means to travel on a flexible schedule as needed, proof of liability and property damage insurance on vehicle used is required.
