Role Summary:
  • The Physical Therapist is responsible for coordinating and providing restorative and rehabilitative Physical Therapy services to participants at the Center and at home, working in close coordination with the PACE Physician, rehabilitation staff and other interdisciplinary team (IDT) members to maximize participant independence and safety and develop or restore function.
  • The Physical Therapist collaborates with the Occupational Therapist to efficiently operate the client rehabilitation department.
  • Physical Therapist is a member of the client management team and facilitates collaborative relationships across the client and with external stakeholders to promote the the client values, team culture and mission.
Duties and Responsibilities:
  • Conduct Physical Therapy assessments to determine the Physical Therapy care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.
  • Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants' care plans including but not limited to providing active and passive exercise, muscle re-education, gait and functional training of ADLs; treatments such as massage, heat, sound, water light or electricity.
  • Supervises the Therapy Assistants including providing training, instruction and guidance needed to implement Functional Maintenance Programs (FMP).
  • Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment.
  • Order, track and manage maintenance for equipment.
  • Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants' condition or functional status. • Work with the other members of the IDT to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.)
  • In coordination with the Marketing Team, support enrollment of prospective participants into the program.
  • Participate in end-of-life care coordination and support.
  • Manage direct reports including training, supervising and mentoring.
  • Be legally authorized to practice in position location, required.
  • Only act within the scope of his or her authority to practice.
  • Complete all required documentation in a timely and accurate manner. • Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
  • Follow the client policies and procedures and participate in any required Quality Improvement activities, staff training and meetings.
  • Communicate regularly with Supervisor and team regarding workload and priorities.
  • Timely completion of all mandated trainings and education.
  • Timely completion of all mandated occupational health screenings as needed.
  • Exercises flexibility in performing assignments as business needs evolve.
  • Other duties as assigned.
Qualifications and Requirements:
Education/Training/Certifications:
  • Bachelor's Degree in Physical Therapy plus five years' of Progressively responsible PT experience.
  • Master's degree in physical therapy or Doctorate in Physical Therapy preferred.
  • Valid California Physical Therapy License, or proof of License eligibility.
  • Valid State Driver's License with a clean DMV record.
  • Copy of recent Vehicle Insurance required.
Skills and Experience:
  • Minimum of two years' Physical Therapy experience in a clinical setting with a frail or elderly population.
