Description

Provides care and services for adults with multifaceted needs that may include stabilization in living environment, coordination of placements, and support and services when discharging from higher levels of care. Services provided are primarily in the community and client's living environment. Services may include skill building, modeling, coaching, coordination and connection to resources and natural supports. Position works within a team to navigate and meet the comprehensive needs of the client.

ESSENTIAL RESPONSIBILITIES
  • Provide regular and consistent services and supports to clients identified appropriate for community supports.
  • Management of all aspects of clinical treatment including referrals to additional resources, crisis assessment and support as needed
  • Work with client on skill building, activities that support stabilization in living environment, connection to community resources and natural supports
  • Service provision shall be conducted primarily in the client's living and other environments as necessary or appropriate
  • Provide outreach and engagement when needed
  • Closely collaborate with other care teams within Polara to coordinate and meet client needs (Inpatient Hospital, Crisis Stabilization Unit, Outpatient Services, Primary Care)
  • Facilitate Adult Recovery Team Meetings (ARTs)
  • Participates as a team member and attends scheduled staffings and supervision.
  • Partners with client and team to identify treatment goals
  • Maintains clinical record per agency policy and procedure.
  • Attends all team meetings, case reviews, staff meetings, supervisory sessions, and in-service trainings
  • Management of all aspects of treatment, including referrals to additional resources when appropriate.
  • Performs other duties as assigned.

Qualifications

  • Minimum Bachelor's degree in related field OR combination of education and experience to equal 4 years in behavioral health will be considered.
  • Must possess valid driver's license
  • Ability to assess situations and demonstrate critical thinking and problem solving abilities
  • Understanding of mental health and impact on functioning
  • Understanding of trauma informed care
  • Experience working with individuals in crisis situations and/or in need of intensive support services
