The licensed clinical social worker (LCSW), as part of the integrated healthcare team, evaluates and manages patients with medical and behavioral health issues within the healthcare setting. The LCSW delivers high-quality mental health consultation to providers and staff as requested. Additionally, the LCSW conducts diagnostic mental health assessments, interventions, education, and referrals for further treatment to patients in the healthcare setting. They may also maintain a small caseload of patients for ongoing therapy in a traditional outpatient setting.

  • Provide consultation and coordinate care of patients with healthcare staff. Identify, refer, and advocate for patients needing specialized behavioral health services and other necessary services.
  • Assist in identifying at-risk patients and developing plans to prevent further psychological or physical deterioration.
  • Offer brief, solution-focused behavioral interventions with active development of a behavioral change plan.
  • Educate patients and families about care, prevention, and treatment enhancement techniques.
  • Participate in meetings and quality improvement activities as required.
  • Assist healthcare providers in recognizing and treating mental disorders and psychosocial challenges.
  • Assess the clinical status of patients referred by healthcare providers through brief consultative contacts.
  • Work with the healthcare team to efficiently and effectively treat and manage patients with chronic and new-onset emotional and/or health problems.
