Nurse Practitioner

<b>Location: </b> Herrin, IL<br> <b>Salary: </b> $95.00 USD Hourly - $99.00 USD Hourly<br> <b>Description: </b> <br>The Judge Group is currently looking for (4) Nurse Practitioners to perform in-home health risk assessments in Southern Illinois MARION. with one of the largest healthcare providers in the country.<br> <ul> <li>Pay rates are EXTREMELY competitive and paid per assessment.</li> <li>Scheduling is based on needs and NP's availability - need about 2-3 days a week.</li> <li>Position can be extremely flexible as a part time position (weekends allowed).</li></ul>Job Description:<br> <ul> <li>Nurse Practitioner&apos;s needed to perform health risk assessments which are essentially history and physicals done within the patient's home or on occasion in nursing home or other health facility OR telehealth</li> <li>Clinical documentation of acute, chronic, and long-term healthcare issues through patient encounter in EMR system;</li> <li>Provider will ensure all appropriate consent forms are signed and dated</li> <li>The provider will educate patients and/or patient's family on chronic medical conditions, preventative care, and medication adherence compliance</li> <li>Provider will complete referrals to case management, nursing, pharmacy, etc. as needed</li> <li>Provider will occasionally need to obtain samples for diagnostic screening tests such as finger stick for HgbA1C, LDL, and Lead levels; urine analysis may also be required based on client need.</li> <li>Provider will be responsible to review patient records for past and present health issues</li> <li>As part of the assessment, the provider must do ADL and Fall Risk evaluation, Depression and Nutritional Health Screening, BMI evaluation, and medication reconciliation </li></ul>Qualifications:<br> <ul> <li>Family NP required.</li> <ul> <li>Can consider Adult if no pediatric patients will be seen.</li></ul> <li>Must be board certified by the ANCC or AANP</li> <li>Must have active/unrestricted NP license</li> <li>New graduates will be considered</li> <li>Must be able to be credentialed by Medicare, Medicaid, and other Private Insurance Companies</li> <li>Must have use of own car with adequate automobile insurance</li></ul> <br>If you or someone you know is interested, please send an updated resume to Steven Kincade at skincade@judge.com.
