LICENSED PRACTICAL NURSE

Candidate to work with family practice physician 4 days per week. Day off is Wednesday and call day is Tuesday.

REPORTS TO: Provider/Director of Nursing
SUMMARY: The Licensed Practical Nurse is responsible for performing and assisting the physician with patient care in an office setting. This role is responsible for managing patient telephone calls, scheduling, call backs, care coordination, refills, assisting with procedures, cleaning and stocking rooms.
DUTIES AND RESPONSIBILITIES:

  • Greets and escorts patients to exam room.
  • Rooms patients for provider exam utilizing established rooming standards.
  • Obtains and documents patient information including vital signs, height, weight, medications, history, symptoms, etc.
  • Gives medications, to include injections, as ordered by the provider.
  • Instructs patients regarding preparation for tests and procedures.
  • Cleans and stocks work areas, including instrument cleaning and sterilization.
  • Manages patient telephone calls appropriately.
  • Calls patients to relay information, obtains information, shares test results, and provides advice/direction from the provider.
  • Manages requests for prescriptions and medication refills following provider guidelines.
  • Schedules special procedures and appointments.
  • Assesses and schedules appointments for patients needing same-day appointment.
  • Refers complex patient questions/situations to an RN or provider.
  • Advises patients utilizing accepted clinical information when provider input is not needed.
  • Monitors laboratory, x-ray, ECG results and other pertinent medical data under provider guidance.
  • Completes referrals using accepted standards.
  • Handles incoming and outgoing mail, assuring the provider sees all necessary information.
  • Speaks to callers, patients, physicians, other staff and anyone they interact with during their assigned hours in a polite, courteous, understandable manner that projects Primary Care Associates' interest in providing high quality customer service.
  • Completes assigned duties in a timely and accurate manner.
  • Demonstrates a team spirit through cooperative, supportive professional work relationships.
  • Assists with procedures completed by provider.
  • Abstracts medical information on patient into EMR.
  • Complies with company policies and procedures.
  • Maintains a regular and reliable level of attendance.
  • Maintains confidentiality of patient information.
  • Performs other related duties as assigned by management.
SUPERVISORY RESPONSIBILITIES:
  • This job has no supervisory responsibilities.
Requirements
QUALIFICATIONS:
  • Associate's Degree (AA) or equivalent from a two-year college or technical school, or six months to one year related experience and/or training, or equivalent combination of education and experience.
  • Certificates, licenses and registrations required:
  • Must have valid, unrestricted LPN license in the State of Wisconsin.
  • Must maintain a current BLS for healthcare certification.
  • Computer skills required:
  • Payroll system; EPIC Software
  • Other skills required:
  • Graduate of a NLN accredited Registered Nursing program.
  • Excellent knowledge of medical terminology, physiology and anatomy.
  • Pleasant and effective communication skills.
  • Ability to work well with all customers of Primary Care Associates.
  • Ability to prioritize and work efficiently.
  • Ability to function as a part of a team with a cooperative attitude.
  • Able to work under pressure and in a fast pace environment.
  • Strong organizational skills.
  • Must have flexibility in work schedule.
PHYSICAL DEMANDS AND WORK ENVIRONMENT:
  • Continually required to stand, walk, bend, stoop, talk or hear.
  • Continually required to utilize hand and finger dexterity.
  • Continually works around fumes, airborne particles, or toxic chemicals
  • Continually exposed to bloodborne and airborne pathogens or infectious materials
  • While performing the duties of this job, the noise level in the work environment is usually loud.
  • The employee must occasionally lift and/or move up to 100 pounds independently.
  • Specific vision abilities required by this job include: close vision; distance vision; color vision; peripheral vision; depth perception and ability to adjust focus.
  • Specialized equipment, machines, or vehicles used: POCT machines (Cepheid;Glucometer).
