Springbrook Center seeks a full time, Monday through Friday, day shift Social Worker! Join the Genesis family as a Social Services Specialist where you will provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life. You will accomplish this by collaborating with patients/residents, their family members/significant other and other nursing center staff to identify patient/resident strengths, social, emotional, and mental health needs and then provide, develop, and/or aide them in accessing those services. Position Highlights *Complete a comprehensive assessment for each patient/resident that identifies social, emotional, and psychological needs and strengths. *Assess each patient/resident for discharge to ensure an appropriate discharge plan is formulated. *Educate nursing center staff on the role of the Social Services and the psychosocial needs of the patients/residents and their families/significant others including the problems of aging and disability. *Conduct patient, family, and staff interviews to ensure Care Area Assessments are completed in accordance with regulation. *Participate in the development of an interdisciplinary plan of care for each patient/resident that identifies the psychosocial needs/issues of the patient/resident, the goals to be accomplished for those needs/issues, and the appropriate Social Services interventions. *Deliver therapeutic interventions to assist patients/residents in coping with their transition and adjustment to a long-term care facility including the social, emotional, and psychological needs. * Facilitate short-term supportive counseling and education to patient/resident and family members/significant others to assist in their understanding of placement and nursing center issues and make referrals to the appropriate Social Service agencies when needed. We have in place multiple testing capabilities including Point-of-Care (POC) testing in every location according to CMS and local guidance. SSS5 *Master's degree from accredited school of Social Work or related field is required. *Must possess any certifications/licensures as required by the State of employment to practice in long-term care. *3-5 years of supervised social work experience in health care setting working directly with individuals preferred. *Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Abuse preferred.
