JOB SUMMARY:

The Lynn Community Health Center (LCHC) Clinical Pharmacist will provide medication management expertise in a Federally Qualified Health Center (FQHC) environment under a collaborative practice agreement. Through direct patient care, embedded on a care team, the Clinical Pharmacist will leverage their clinical expertise and knowledge of medications to ensure our patients have better health outcomes. The Clinical Pharmacist will work closely with the Community Pharmacy Cooperative (CPC) team, which manages our internal pharmacy. CPC is a subsidiary organization of Community Care Cooperative (C3), the Accountable Care Organization of which LCHC is a member. The Clinical Pharmacist position is currently embedded at the practice site though some responsibilities may be achievable remotely.

PURPOSE: To improve patient outcomes and safety.

WORK HOURS: 40 HOURS FULL TIME HYBRID

RESPONSIBILITIES:

Provides collaborative drug therapy management (CDTM) including initiating, modifying, discontinuing, monitoring, refilling drug regimens for specific populations based on organizational needs.
Maintains a collaborative practice agreement and Massachusetts Controlled Substance License consistent with state law for purposes of providing CDTM.
Educates patients on their medications and promotes effective medication use
Identifies patient specific barriers to adherence and develops plan to improve medication-taking behavior (ex. prepackaged medication packs, delivery, med synchronization, coordination with inhouse pharmacy)
Assists in addressing medication access issues related to affordability
Understands the interdependencies of all pharmacy team member responsibilities and collaborates with retail pharmacists working towards a seamless pharmacy experience for patients.
Documents encounters and interventions in our Electronic Health Record, maintaining patient confidentiality in all aspects of care including use of electronic information.
Participate in collection, maintenance, aggregation, and presentation of data to support clinical pharmacy services.

QUALIFICATIONS AND EXPERIENCE:

Graduate of an ACPE accredited college of pharmacy with a Doctor of Pharmacy degree. Prefer ASHP accredited residency. (BPS) Board Certified in a Pharmacy Specialty
Licensed pharmacist in the state of Massachusetts. Adhere to the requirements of the Massachusetts Board of Pharmacy for licensed pharmacists.
Maintains required continuing education for licensure.
Completed ASHP Accredited PGY1 with Emphasis in Ambulatory Care or PGY 1 plus PGY 2 in Ambulatory Care preferred.
3-5 years of clinical pharmacy experience preferred.
Board Certified preferred (BCACP) certified or equivalent certification in area of expertise or willingness/ability to obtain within 2 years of employment preferred.
