Presbyterian is hiring an

RN Care Manager

to work at the Family Medicine Clinic on San Mateo

The

Care Manage r pro-actively manages the care for patients along a continuum by facilitating collaborative care between patient, family/support system, physicians, and other ancillary departments/members of the primary care team

Qualifications Credentials:

Registered Nurse

( RN ) -

State of New Mexico

or

Compact State Nursing License

required. BLS certification within 90 days of hire.

Education:

Associate Degree in Nursing

Prefer experience
