Min Max Overview Presbyterian Homes & Services is seeking a Registered Nurse to join our RAI/MDS team. This position would be primarily day shift hours (flexible if it still meets business needs), completed during a Monday-Friday work week (no weekends). No on-call requirement and job duties do not extend outside of MDS role. You will support designated sites in this role, but are able to office at a PHS location convenient to you (provided there is office space available at that location). Responsibilities The MDS Nurse is responsible for completing, coordinating, implementing and monitoring the comprehensive RAI-Resident Assessment Instrument (MDS) and corresponding assessments, and developing resident care plans consistent with regulations and established clinical best practices. * Working closely with the RAI Director to complete MDS assessments for assigned residents. * Completing the MDS, CAA's (Care Area Assessments), and comprehensive care plans accurately by following the RAI guidelines and within required time frames. * Consulting with interdisciplinary team members for pertinent clinical information. * Utilizing chart audits, resident/remily interviews and communication with direct care staff when completing documentation. * Attending daily IDT phone calls * Ensuring care plans accurately reflect the cares provided. Qualifications * Registered nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices. * Prior working nursing experience in long term care, transitional or acute care. Previous successful MDS experience preferred. * Demonstrated compatibility with PHS's mission and operating philosophies. * Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
