Community Registered Nurse (RN) - $2,500 Sign On Bonus

Angels On Call Homecare is searching for a talented and experienced Registered Nurse for the open position of RN Case Manager. A minimum of one year in homecare experience is a must. This is a fulltime position and the availability is Monday through Friday from 9am-5pm with weekly on call hours on one weekday per week and one - two weekends per month. Case management work would involve traveling throughout Bronx, Westchester, Putnam, Dutchess, Sullivan, Orange, Rockland, and Ulster counties and working with client families of Angels on Call, including assessments / drafting care plans / training home health aides. The primary workstation can either be our main office located at 126 Union Avenue in Peekskill, our satellite office at Kendal on Hudson in Sleepy Hollow, or our satellite office at The Fountains in Mill Brook.
This position has a starting salary of $115,000.00 which can be increased up to $130,000.00 based on relevant experience.

Qualifications:
  • Current New York State RN license
  • Minimum (1) year recent and verifiable experience with home care agency.
  • In Home Care or Direct Care experience also preferred.
  • Recent Case Management experience (Preferred)
  • Willing to meet with and speak with patients, their family, and their medical professionals to develop, update and maintain proper care plan
  • Strong assessment and organizational skills
  • Prior supervisory experience with field staff, experience with clinical oversight, monitoring quality assurance, and overall case management
  • Demonstrated successful track record of working independently
  • Ambition to grow and advance beyond current position
  • Strong communication and computer skills
