Licensed Practical Nurse (LPN)
Job Summary: The Licensed Practical Nurse (LPN) is a vital member of the healthcare team responsible for providing direct patient care under the supervision of a Registered Nurse (RN). As a PRN (Per Diem) LPN, you will work on a flexible schedule to assist with various nursing duties, ensuring the comfort, safety, and well-being of patients.
Key Responsibilities:

  • Administer medications and treatments as prescribed by the physician
  • Monitor vital signs, including blood pressure, temperature, pulse, and respiration.
  • Provide wound care, ostomy care, and catheter care as needed
  • Assist with daily living activities, such as bathing, dressing, and feeding.
  • Observe patients for changes in condition and report any significant findings to the RN.
  • Accurately document patient care activities, including medication administration, vital signs, and treatment procedures.
  • Maintain patient records and charts in accordance with facility policies and regulations.
  • Adhere to all facility policies, procedures, and regulations, including infection control and safety standards.
Qualifications:
  • Current LPN license in Ohio
  • BLS Certification.
  • Strong clinical skills and experience in a skilled nursing facility preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
  • Compassionate and patient-focused attitude.
