OVERVIEW OF THE ROLE/JOBThe primary responsibility of your position is to provide care to the Villagers living in a home by protecting, sustaining and nurturing the Villagers, providing assistance with activities of daily living and meeting other needs as required. The core goal of the Village Care Provider's (CNA)work is to provide assistance in a manner that promotes a high quality of life and a positive community. All responsibilities will follow the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities. You are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.KEY RESPONSIBILITIES:Working with your household team of Village Care Providers (CNA), recognize and respond to the needs and desires of the villagersWork various houses or work flexible schedules as agency and supervisor deems necessary. Staff attendance and promptness to assigned shifts is essential. The responsibilities of this position may require staff to be able to work irregular, extended, and/or be available for on-call work hours, evenings, nights, weekends, holidays, etc.Promote autonomy of villagers in decision-making regarding their daily lives and the life of the houseWork together with the other Village Care Provider (CNA) as a self-managed team, responsible for managing all aspects of the home, including food ordering, preparation and clean up, light housekeeping duties, all laundry needs of the villagers, and assisting villagers in engaging in the daily activities of their choosingCooperate with the nurses and other clinical team members to ensure the well-being of each villagerReceive and report resident status at the beginning and end of each shift and as needed to team members and to nurses and clinical support team membersAssist villagers with personal care functions, including: dental and mouth care, bathing, dressing and grooming, hair care, nail care, shaving, bowel and bladder care, other personal and hygiene careAssist with and/or perform restorative and rehabilitative procedures as outlined and assigned by the care planMeasure and record vital signs such as temperature, pulse, respirations, weight, height, and anatomical dimension/circumferenceAssist moving, positioning and transporting villagers into and out of beds, chairs, wheelchairs, lifts, etc.; make room checks and verify resident locationAssist with the application of slings, elastic bandages, binders, etcProvide range of motion exercises as outlined and assigned by the care planWork with the villagers and the Dietitian to create a balanced menu that meets the nutritional needs of the villagers as well as their preferences for foods they enjoyPrepare and serve meals, between meal, bedtime and other nutritional snacks per the posted menu and stated preferences of the villagersKeep all areas of the home clean, performing routine housekeeping duties (i.e. changing bed linens, emptying trash, dusting)Launders clothing, linen and other articlesAssures villager calls for assistance are answered promptlyCooperate with the Activity professional to develop and carry out support of the villager's daily activities, including opportunity for spontaneous activities to occurMaintain confidentiality and rights of villager, family, employee, and house information in compliance with PHI HIPAA policies and applicable regulationsMeets with the Clinical Service Team as schedule or needed to discuss villagers' needs and progress in identifying and correcting problemsObserve, monitor, and report to the CST any change in conditionDocument observed data per policyWill serve in different coordinator roles on a rotating basis. These include Team, Care, Scheduling, Food & Housekeeping CoordinatorAttend and participate in Team meetings on a regular basisAttends and participates in orientation programs, on-going training and educational classesMaintains all equipment and appliances in the house by adhering to proper operating guidelinesDocuments and maintains records necessary to comply with all state and federal regulationsReport any problems with equipment or appliances immediately to the maintenance directorAdheres to all safety guidelines and processes; and observe, monitor, and intervene in or report to the clinical support team any unsafe conditions; observes standard precautionsReports accidents and incidents when they occurReport any abuse or suspected abuse to the Abuse Coordinator immediatelyFollows established smoking regulations and reports violations. Six months experience in a long-term care environment preferredMust possess a current, unencumbered, active CNA certification to practice in the statePHYSICAL REQUIREMENTS:Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.Must meet the general health requirements set forth by the policies of this facility, which may include a mental and physical physical and sensory requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Is subject to frequent interruptions. Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.Is subject to difficult and emotionally upset residents, family members, etc. Communicates with other staff, clinical team, and medical director. May work beyond normal duty hours, on weekends, and in other positions temporarily, when necessary. Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants. Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses. May be subject to the handling of and exposure to hazardous chemicals.
