• Must have an active state license
• Does the Provider need to prescribe? Yes
• Is there a state specific CSR that will be needed? No
• Certifications required: BLS, DEA, & COVID Vaccination
• Background Experience: Minimum one (1) year practicing dentistry; correctional experience preferred (not required); and supervisory experience preferred
• Credentialing: 30 Days
• Start Date: ASAC
• Dates: 3 Months from Start Date with possibility of extension
• Work Schedule: 40hrs per week
• Outpatient clinic within corrections facility
• Patient Volume: 10-15 PPD
• Staffing Mix: Hygienists, Dental Assistants, Administrative Staff
• EMR: CorrEMR - Correctional Specific EMR System
