As we continue to expand our team, we are seeking a skilled and dedicated Licensed Practical Nurse (LPN) to join us in providing high-quality home care services.

Essential Job Functions of a Home Health Care Nurse:

Perform home health LPN (Licensed Practical Nurse) duties in patients homes

Administer medications, perform wound care, and provide other skilled nursing services in accordance with physician orders.

Develops and implements nursing care plans and discharge plans with involvement of patient and patient care team.

Review consumer medical documentation and or health referral forms, as relevant to the case, and adjust care plans are needed.

Health promotion, teaching, and training of family members

Understands and demonstrates commitment to the company's mission through daily communication with participants, families, community, and staff.

Certificates, Licenses, and Registrations:

Current Massachusetts LPN Licensure

All state required training and certification completed in mandated timeframes.

Drivers License / Reliable means of transportation

Qualifications:

Good Physical Assessment skills.

Must be able to use and understand electronic mobile devices for documentation and other electronic applications for the job

Effective Time Management

Travel Required

Physical Requirements:

Heavy Work. Exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.
