General Summary of Position
Collaborates with the healthcare team, community agencies, and primary care providers to ensure continuity of the patient's care through outpatient healthcare settings. Promotes effective communication among healthcare team members, including the patient, family, and significant others. Facilitates instruction to the patient and family based on identified learning needs. Calls and or visits patients at home for follow-up as needed. PLEASE NOTE: THIS POSITION SHALL OFFICIALLY BE KNOWN AS "CARE COORDINATOR" TO THE COMMUNITY.


Primary Duties and Responsibilities

  • Contributes to the achievement of established grant goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations.
  • Administers direct patient care and healthcare provider support.
  • In collaboration with healthcare providers, clinical and administrative staff creates clinical pathways and protocols for community care management.
  • Establishes strong working relationships with community health and human service providers, including establishing a data base of services and establishing referral networks.
  • Functions as a clinical resource to healthcare practitioners, clinical staff, outside patient providers and patient/family and initiates referrals to appropriate support services.
  • Demonstrates knowledge of and participates in the development and promotion of clinical practices following MedStar St. Mary's Hospital Standards of Care.
  • Actively participates in Quality Improvement Processes as a basis for assisting with the implementation of changes in practice, when needed. Collects and analyses data as needed.
  • With hospital Case Management services, establishes transition plans for patients who are being discharged from the hospital.
  • Participates, and demonstrates competency in, Electronic Medical Record (EMR) initiatives.
  • Establishes and implements care plans for health promotion, illness prevention and for disease self-management for all patients referred by the HEZ or other providers.
  • Assess protocols for all required case management indicators within required timeframes.
  • Documents complete and clinical relevant assessments, reassessments, plans, interventions and evaluations in the EMR.
  • Performs registered nurse functions for providers as needed.

    Minimum Qualifications
    Education

    • Bachelor's degree Nursing required or
    • completion within a year of hire date. required

    Experience

    • 3-4 years Clinical experience in a related area. required and
    • 1-2 years Experience in case management or similar role required and
    • 3-4 years Experience in case management or similar role preferred

    Licenses and Certifications

    • RN - Registered Nurse - State Licensure and/or Compact State Licensure Current Maryland RN license or license eligible. required


    This position has a hiring range of $41.98 - $71.68
