RN/LPN - New Mercer Commons Assisted Living

New Mercer Commons Assisted Living is located at 900 Centre Ave. Fort Collins, CO and serves a wide range of needs by offering three unique sections of care: Assisted, Assisted Plus, and Memory Care all located within one building. We cater to individualized needs by offering seven styles of apartments ranging from Studios to Royal Suites.

Compensation:

• Wages start at $23.00-$36.00/hr. depending on experience and licensure
• Shift and weekend differential pay

Work Schedule:

• Part-time
• 24+hours per week
• 11:00 am - 7:30 pm

RN/LPN

• Conduct comprehensive assessments of residents' health status
• Develop and implement individualized care plans based on assessment findings
• Administer medications and treatments according to prescribed protocols
• Monitor residents' vital signs and health condition, and report any changes to the physician or supervisor
• Communicate effectively with residents, families, and interdisciplinary team members
• Coordinate care with physicians, therapists, and other healthcare providers
• Provide guidance and support to caregivers and other staff members
• Participate in care conferences to review resident care plans and progress

Qualifications

• For RN: Bachelor's or Associate degree in Nursing (BSN or ADN) from an accredited school of nursing and currently licensed in the State of Colorado
• For LPN: Completion of a state-approved practical nursing program and current LPN license in the State of Colorado
• Current CPR certification
• Experience in geriatric care or assisted living preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work effectively as part of a team and independently
