Assesses, plans, organizes, and administers rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.
VMRC Therapy team has recently transitioned from a contracted team to in-house. We are currently seeking PRN DISCIPLINE to help complete our therapy team. As a vital team member, you will have the opportunity to support residents in meeting their therapy goals and improving their quality of life.
Job Qualifications:
  • Education: Graduate of an accredited therapy program
  • Experience: 2-3 years of related experience; or equivalent combination of education and experience (preferred).
  • Licenses/Certifications: Current Physical Therapist state license; CPR certification
Requirements
  • Restores patient's function, alleviates pain, and prevents disabilities by planning and administering medically prescribed physical therapy.
  • Provides quality care by assessing and interpreting evaluations and test results.
  • Develops Plan of Treatment for each patient based on initial and ongoing evaluations including short- and long-term goals, collaboration demands of other staff members, and required patient and caregiver involvement.
  • Determines physical therapy treatment plans in consultation with physicians or by prescription.
  • Implements Plan of Treatment on a consistent schedule and adjusts this schedule and plan according to patients progress.
  • Helps patient accomplish treatment plan by administering manual exercises, giving massages, and using equipment as necessary.
  • Evaluates mobility, strength and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and realistically attainable.
  • Evaluates and records patients progress, modifying treatment plans, and trying new treatments if required.
  • Evaluates the use and fit of prosthetic and orthotic devices by observing, and noting patient's progress, and recommending adjustments and modifications.
  • Documents patient care services according to approved medical documentation and facility documentation procedures.
  • Reports patient progress to patient, patient caregivers and loved ones, facility staff and healthcare professionals involved in ongoing treatment/care of patients.
  • Develops a plan for patients following completion of their Plan of Treatment with appropriate in-home care services.
