RESPONSIBILITIES:
As a Mid-Level Provider with Primary Care Solutions, you'll have the opportunity to serve our nation's Veterans with the same exceptional level of care our Veterans have shown while serving our country. We are seeking healthcare professionals who are driven and committed to ensuring the health and well-being of our nation's Veterans.
  • Variable remote and onsite schedule based on the needs of the clinics.
  • Minimum requirement of 2 weeks per month on site at a clinic.
  • Office hours are Mon - Fri 8:00am to 4:30pm.
  • This is a Full-Time position, and we are looking to hire 2 individuals.
  • Examine the patient/client and determine the best course of action while documenting in our paperless health program - The VA's VISTA/CPRS electronic patient record system.
  • Develop and implement patient management plans, communicating with Patient Aligned Care Team (PACT) to ensure appropriate standards of care.
  • Discuss patient outcomes, policies, procedures, and records with Medical Director.
  • Completes all clinical reminders to keep each patient up to date in the system.
QUALIFICATIONS:
  • Graduate of an accredited school of nursing and graduate of an accredited program for nurse practitioners (MSN required).
  • Board-certified in family practice or internal medicine
  • Certification by an applicable professional organization
  • 1 year of clinical medical practice preferred
  • Experience with VISTA/CPRS electronic patient record system is required
  • Current, unrestricted license in the state of desired employment
  • Licensed in AZ, NM, TX, USVI within the first 6 months of hire date
  • Current BLS for providers from the American Heart Association (to be renewed annually)
  • Current, unrestricted Drug Enforcement Administration (DEA) registration
  • Excellent computer skills
