Health Information Clerk

HOURS: Full-time, Mon-Fri, 8am-5pm

RESPONSIBILITIES:

Retrieves medical records and loose sheet medical reports from clinical and ancillary areas on a daily basis. Abstracts, assembles, and analyzes for completeness inpatient, outpatient, same day surgery, emergency room and geri-psych unit records in accordance with regulatory rules and hospital policies. Generates physician delinquent record count. Sends physicians with delinquent deficiencies and e-signs warning letters. Scans and electronically converts all paper portions of the patients' medical record into the hospital system. Produces reports and prints records utilizing the hospital's computer system. Faxes information electronically and manually. Releases information in accordance with hospital, HIPAA, privacy, and security rules and regulations. Maintains strict patient and hospital confidentiality in all matters. Monitors physicians' voice recognition documents and assure completion and distribution. Answers telephone promptly and with courtesy. Participates in all hospital and departmental QI projects assigned. Special assignment or any other clerical duties assigned by the Manager of HIM.

PROFESSIONAL REQUIREMENTS:

Must be knowledgeable in federal and state laws and hospital policies that pertain to the release of confidential information and HIPAA security. Must be familiar with the operation of office equipment. Computer experience required. Ability to type accurately at a minimum of 40 words per minute. Must be knowledgeable about medical record content and completion requirements. Possess filing proficiency. Possess good telephone and public relations etiquette

EDUCATION:

High school graduate, or equivalent, with emphasis on Microsoft office software and office/medical office procedures.

EXPERIENCE:

Experience in analyzing and interpreting data. Medical terminology preferred.
Typing experience required with computer experience preferred. Office or medical setting helpful.
