Licensed Practical Nurse (LPN) -- Oak Harbor & Perrysburg Full-Time Position -- Day Shift Monday thru Friday, Must be willing to travel Salary is competitive and based on experience. Job Summary -- We are looking for a compassionate and committed Licensed Practical Nurse (LPN) to become a valued member of our team. In this role, the LPN will deliver high-quality nursing care to our individuals under the guidance of a Registered Nurse (RN). The ideal candidate will possess strong clinical expertise, outstanding communication skills, and a deep passion for working with individuals with intellectual and developmental disabilities. Key Responsibilities: Administer medications and treatments as prescribed by physicians. Monitor patients' vital signs and report any changes to the healthcare team. Assist with patient assessments and the development of care plans. Provide wound care, including dressing changes and monitoring for signs of infection. Support patients with daily activities, such as bathing, dressing, and mobility. Document patient care and progress accurately in medical records. Collaborate with RNs, physicians, and other healthcare professionals to ensure optimal patient outcomes. Educate patients and their families on health conditions, medications, and home care instructions. Maintain a safe and clean environment in compliance with healthcare regulations. Participate in continuing education and professional development activities. Qualifications: Current and valid LPN license in the state of Ohio. Graduation from an accredited LPN program. Minimum of 3 years of clinical nursing experience preferred. Excellent communication and interpersonal skills. Ability to work effectively in a team-oriented environment.
