Provides intensive case management, group counseling, and crisis intervention in collaboration with partners providing medical care coordination combining Integrated Healthcare with Housing First. Clients have a history of chronic homelessness and disability – including a high number of individuals with dual diagnoses of substance use disorders and serious mental illness.
Utilizes the Transtheoretical Model and Motivational Interviewing counseling style. Also utilizes CBT in specialized applications for persons with MDD/Depression, Bipolar disorder, anxiety disorders, and/or substance use disorders as appropriate. Participates in recurring interdisciplinary treatment team meetings with health care professionals.
Offices on-site with our partners at one of many nearby Housing Facilities we collaborate with locally; can be a solo placement or team office share depending on the project. Related Master’s Degree in Social Work or Counseling preferred, candidates desiring to work towards a LCSW or LPC ideal. Strongly prefer 2 years case management with dual diagnoses populations. Related Bachelors Degree Required.
