Community Care is hiring a Case Manager/Care Manager to join our Outagamie Program team This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. Community Care's mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. It is based in Outagamie County with telework currently being the norm. Case Manager/Care Manager Job Responsibilities: * Manages an assigned caseload of program members in collaboration with an RN Care Manager * Completes comprehensive member assessments, member centered care plans, & functional screens * Provides ongoing care coordination and quality assurance monitoring of services * Maintains required documentation in adherence to established standards. Case Manager/Care Manager Job Requirements: Education: Bachelor's Degree in a Human Services Area required. Experience:One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity required. Certification:Eligibility for long-term care functional screener certification is required Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage. Care Manager Job Schedule: * Monday to Friday * Generally during business hours of operation (8AM-4:30PM) Community Care is a nonprofit organization with over 40 years of experience helping adults with long-term care needs live as independently as possible.
