In this role, you will deal directly with our participants and the guardian to find out what barriers they are facing, how we may improve our services, and gaining an understanding of why services ended or why services were further declined after the crisis event.

EDUCATION and/or EXPERIENCE
High School diploma or GED and one to two years' experience working with at-risk youth. Bachelor's degree (BA/BS/BSW) preferred.

- Review files for service background on participant for accurate follow up information.
- Gathering information related to participant and guardian complaints, barriers, or concerns.
- Develop a plan with participant and guardian for service continuation.
- Working with ICBS Staff to coordinate intakes and follow up sessions with participants and guardians.
- Participate in ICBS events and outreach.
- Creating and maintaining data reports related to participant and guardian complaints, barriers, or concerns.
- Building positive relationships with participants and guardians
- All other duties as assigned.

- Valid Illinois driver's license with a clear driving record.
- Documentation verifying current automobile insurance.
