School Based CM/Region 1- Emergency Youth Crisis Counselor

The School Based Case Manager/Region 1-Emergency Crisis Counselor is a hybrid position that promotes and supports individuals in the school setting who are at risk of developing or have mental health and/or substance abuse issues. Services include assessments, supportive level counseling, advocacy, and link/referral to needed services as indicated in the ISP. This position is dedicated to providing quality service adhering to safety regulations and gaining the knowledge to assist consumers with any and all needs. Additional services include preadmission screenings, assessing crisis situations, providing de-escalation strategies, supportive level counseling, advocacy, and link/referral to needed services as indicated in the individual's ISP. The incumbent will have ongoing support and clinical guidance from a licensed/licensed eligible supervisor.
REQUIREMENTS

  • Bachelor's Degree in a relevant Human Services field (required)
  • QMHP-C or QMHP-Trainee (required)
  • Valid Driver's License (required)
RESPONSIBILITIES
  • Provides ongoing case management and behavioral interventions to clients, based on assessed needs and goals of clients
  • Assists, supports, monitors, and intervenes with clients in the day treatment program. Implements behavioral management programs as outlined in the service plan in an ethical and competent manner.
  • Develops Individual Service Plans based on assessed needs, in consultation with supervisory staff, within the team model of service delivery.
