Job Description:
The Hospice RN Case Manager is responsible for delivering professional nursing care to patients in accordance with the established plan of care. This role involves performing comprehensive assessments, formulating individualized plans of care, providing skilled interventions, ensuring thorough documentation, and delivering patient/family education. The Case Manager also supervises and evaluates staff, ensures compliance with regulations, and contributes to policy improvements.
Requirements:
- Graduate of an accredited school of nursing, Bachelor's degree preferred
- Current Registered Nurse licensure (in good standing) in the state(s) of practice
- One (1) year experience in hospice care or post-acute setting
- Current CPR certification
- Valid driver's license and automobile insurance
