Requirements
The MDS Coordinator is responsible for the preparation of all MDS assessments, timeliness and thoroughness, electronic submission of assessments, coordination of information to accounting and oversight of the census by performing the following duties.
  • Prepares all Medicare-A and non -Medicare MDS and OBRA assessments timely and completely.
  • Reviews all assessment associated paperwork and oversees accuracy of the census.
  • Responsible for electronic submissions of assessments to appropriate government agencies.
  • All Team members will adhere to the regulatory deadlines
  • The MDS Coordinator will address all MDS exceptions, triggers and individualized Care Plans.
  • Communicate appropriate information to Accounting Department and oversees the accuracy of census.
  • The MDS Coordinator will organize, coordinate and conduct the Care Plan Meeting.
Expectations : Timely completion and submissions of all MDS's; Care plan /family and resident meetings; care plan development; knowledge of payor sources and managing of continued authorizations with case managers; knowledge of insurance denial and appeal process.
Education :
  • RN with a minimum of five years of experience with MDS responsibilities; PDPM knowledge is a must; long term care charge nurse experience preferred.
  • MDS certification is a plus
