Overview
HarmonyCares is seeking a Speech Therapist to provide speech therapy services in the comfort of patients' homes. Our team is dedicated to delivering high-quality healthcare and personalized support to individuals who face challenges in accessing care. Responsibilities
- Assess and evaluate patient needs and establish care goals
- Provide speech therapy services and report patient status to the clinical supervisor and referring physician
- Document daily changes in patient care plans and attend team conferences and educational in-services as required
- Perform OASIS time point assessments and submit recertification paperwork in line with Medicare criteria
Qualifications
- Current unencumbered State Professional License for speech therapy
- One year of experience as a professional Speech Therapist, preferably in a home care setting
- Competency in comprehensive assessment and OASIS-C
- Sound professional clinical judgment and proficiency in clinical skills
- Proficiency in Microsoft Office
