(DSI) Job Description POSITION TITLE: Nurse  - Wellness Coordinator DIVISION: Nursing SUPERVISOR: Director of Nursing CLASSIFICATION: Non-Exempt PURPOSE: As an LPN, you'll play a pivotal role in enriching the lives of our clients, fostering their well-being in both group home and community-based waiver settings. Beyond routine care, you'll orchestrate healthcare coordination, collaborating closely with clients, their families, and healthcare providers to craft personalized plans tailored to their specific needs and aspirations. Your expertise will extend to mentoring staff through dynamic Core A/B medication administration classes, equipping them with comprehensive wellness care strategies. Additionally, you'll be the go-to resource for swift consultation on any unforeseen client health concerns, ensuring seamless support at all times. KEY AREAS OF RESPONSIBILITY: * All service standards for the Wellness Coordination program are adhered by. * Develop and oversee the maintenance and routine monitoring and training of Waiver participants Wellness Support plans per the Wellness Standard guidelines. * Complete face to face assessments in accordance with the Wellness Tier Guidelines. Consults as determined by current healthcare needs. * Complete Wellness Quarterly reports * Follow treatment and medication plans for clients as recommended by a physician. * Assist in the scheduling of medical appointments. Attend medical or specialty appointments when necessary. * Maintain accurate medical files for clients. Prepare and maintain other records pertaining to medical information. * Provide direct care and first aid to clients. * Plan and present Infectious Disease Training for employees as necessary * Maintain great communication and professional relationships with clients, family members and co-workers. * Monitor, order, and stock medical supplies. * Coordinate/ administer physician ordered injections/inoculations as needed. * Review of Medication Administration record/Physicians' orders * Coordinate/conduct medical in-services as needed. * Must meet physical requirements. Physical requirements may include but not limited to bending, reaching, stretching, lifting 25-50 pounds, running, walking, squatting, assistance with physical transfer, handling physical aggression as defined in CPI training. * Teach agency Med Core A & B classes as needed. ESSENTIAL JOB SKILLS: * Excellent and professional communication skills, both oral and written. * Must possess critical thinking skills. * Complete other duties and responsibilities as assigned. * Comply with ethical standards of DSI, FSSA, and other governing bodies. * Ability to work flexible, non-standard hours, including occasional evenings and weekends. * Must possess excellent telephone skills. * Ability to effectively operate equipment such as a scanner and copier and have a working knowledge of computers and related software (i.e. Microsoft Office) REQUIREMENTS: * Licensed Practical Nurse * Maintain CPR, TB, First Aid and CPI certifications * Must have excellent oral and written communication skills and maintain confidentiality with sensitive information. * Maintain a valid Indiana driver's license, insurance on personal vehicle (minimum of PLPD with medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier.
