Duties Description POSITION DESCRIPTION:The Licensed Master Social Worker (LMSW) serves as the Clinical Assistant to the Developmental Disabilities Program Specialist 4 (DDPS4) in Orange county. Specific responsibilities for the LMSW include working with the residential teams to screen new individuals for admissions (in coordination with the placement coordinator), to prepare for new admissions, and support those individuals through the transition period. The LMSW will work collaboratively with the Intensive Clinical Support Team (ICST) for individuals with the highest level complex behavioral need and will serve as a conduit from the clinical teams to the DDPS4, to ensure recommended supports are implemented.Duties and Responsibilities:In order to accomplish this function, the candidate for the position requires that the LMSW be able to work largely independently but in collaboration with the DDPS4 to accomplish the following tasks:1. Serves as a member of a multidisciplinary treatment team; participates in team meetings, presents clinical information and observations, and provides consultation and assistance to staff in other disciplines; and coordinates care and treatment planning for individuals.a. Serve as a clinical consultant to multiple treatment teams within the assigned area.b. Attend Life Plan meetings and monthly res-hab meetings for high needs and/or dually diagnosed individuals, as appropriate/needed.c. During scheduled and unscheduled site visits, observe and document any changes in the individuals’ clinical stability and presentation. Communicate findings, clinical assessment, and recommendations to the treatment team and the DDPS4 for follow up.2. Provides a variety of social work and treatment services to individuals and families that include life skills, vocational and career training, family support, and community integration that assists individuals in achieving maximum participation in work and socialactivities.a. Plan, organize and oversee community activities in high needs group homes. Educate and train evening shift supervisors and house managers on community activities and other HCBS regulations. Evaluate the success of community activities and implement changes as appropriate. Ensure the timely, accurate and thorough documentation of community activities.b. Plan, organize and run a group focused on developing independent living skills, exploring/preparing for a lower level of care and/or community vocational opportunities.c. Oversee the completion and documentation of self-advocacy activities. Work with teams to identify and prepare individuals to participate in Incident Review Committee, interviews, etc.d. Be a part of the Home and Community Based Services (HCBS) committee. Oversee the implementation of multiple HCBS standards.3. Develops effective treatment plans in conjunction with individuals and treatment teams; monitors and evaluates individuals’ progress; and modifies treatment plans or terminates services when . Liaison with the treatment team and placement coordinator to identify individuals who are appropriate for a lower level of care with voluntary agencies or more independent living settings.b. Liaison with the placement coordinator regarding agency resources and bed availability to match referrals with available beds.c. Coordinate and support individual’s discharge to out of state placements.4. May supervise and train social work interns, and paraprofessional social work and/or direct care staff.a. Oversee multiple teams’ completion of a variety of reports, training, and initiatives. Report on training compliance. Develop interventions to successfully complete training . Educate and oversee initiatives including overtime use of direct care staff to ensure quality of care.5. Prepares and maintains documentation and records, including progress notes, observations, recommendations, and discharge plans.a. Liaison and partner with treatment team leaders to complete monthly reports to oversee the care and treatment to individuals.b. Support and assist Habilitation Specialists and direct care staff with accurate billing . Oversees the completion of operational and clinical documentation within IRA’s as needed.d. Attend traineeship council and provide consultation to the committee to ensure the successful completion of traineeship. Work with house managers and supervisors to ensure the completion of performance evaluations.

Additional Comments ADDITIONAL COMMENTS:*Please note, the posted Hiring Salary includes the sum of the statutory hiring rate for ASU Grade 20 ($72,032), along with any special salary treatments in effect for this position in this location, such as a Mid-Hudson Adjustment ($1,650).
