View Point Health Job Title: Mobilizing "MATCH" Navigator (Care Coordinator) Job Code: SST012/ Social Service Technician 3 Shift: Full-Time (40 hours per week on average) Job Hours: Times May Vary Base Location: Region 3 (Gwinnett, Rockdale, Newton, Fulton, DeKalb, and Clayton Counties) Division/Department/Program: SOC - MATCH View Point Health has partnered with the Georgia Department of Behavioral and Developmental Disabilities (DBHDD) to focus on assisting youth in Region 3, ages 6-21 years of age, who are experiencing health crisis and barriers to accessing appropriate care and support, throughout a continuum of care. This partnership allows us to implement the required and allowable activities by studying, developing, and piloting a formalized multi-agency care coordination protocol focused on removing barriers and expediting access to the right care at the right time for youth and families. This position offers support, education, advocacy, case management and other assistance to at-risk individuals to identify and remove barriers to obtaining community-based treatment. This position will also provide consultation and expertise to ensure the course of care is coordinated, scheduled, and communicated through the program process to confirm all program participants are supported appropriately. Duties &amp; Responsibilities: Develop and maintain a current working knowledge of services available in the community provider network in Georgia Department of Behavioral and Developmental Disabilities (DBHDD) Region 3 (Gwinnett, Rockdale, Newton, Fulton, DeKalb, and Clayton Counties). Develop and maintain working relationships with English and Spanish speaking community partners, employers, parole / probation officers, social service and child welfare agencies, schools, hospitals, housing providers, 24/7 mental health crisis services, transition age services, residential and crisis treatment facilities, parent and family organizations, trauma and grief-informed care providers, peer respite centers, community behavioral health providers, local nonprofits and other resources that support youth from Georgia Department of Behavioral and Developmental Disabilities (DBHDD) Region 3 to facilitate services for individuals and families. Travel up to 50% required, developing and maintaining relationships with community partners, employers, parole/probation officers, social service and child welfare agencies, schools, hospitals, housing providers, 24/7 mental health crisis services, transition age services, residential and crisis treatment facilities, parent and family organizations, trauma and grief-informed care providers, peer respite centers, community behavioral health providers, local nonprofits and other resources that support youth from Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) Region 3 to facilitate services for individuals and families. Increasing Intensive Customized Care Coordination (IC3) enrollment for youth and young adults meeting eligibility criteria and evaluating outcomes (e.g. linkage, enrollment, timelines, recidivism). Supporting pre-enrollment care coordination support to IC3 and facilitating referrals and linkage to services and supports for youth not meeting IC3 eligibility criteria. Help develop or update Behavioral Health Assessments (BHAs) for IC3 triaged and / or enrolled youth and young adults. Provide and / or coordinate individualized evidence based and culturally appropriate mental health and substance abuse assessment resources for individuals referred. Develop action plans to address identified service barriers and gaps for identified youth, young adults, and families. Provide continuum of care resources through both internal and external service providers and resources. Develop and maintain relationships, contracts and referrals with statewide therapeutic foster care and respite providers. Support statewide respite and therapeutic foster care programs and ensure contract and compliance with MATCH subcontractors including facilitation of operations, accounting, and provider services. Develop and maintain relationships, contracts, and referrals with partner agencies about to support youth, young adult and family needs including, but not limited to, family support organizations, community-based expressive services, historic and cultural experiences. Support MATCH referral process into the comprehensive Georgia Department of Behavioral and Developmental Disabilities (DBHDD) network (including IC3) and market services and resources throughout the statewide continuum of care. Coordinate Intensive Outpatient services and reimbursements with contracted partner agencies. Support Psychiatric Residential Treatment Facilities (PRTFs), Crisis Stabilization Units (CSUs) and Psychiatric Hospitals in community transition planning for youth and families in Region 3. Study and refine PRTF and Division of Child and Family Services (DFCS) referral pathways, in addition to developing, studying, and piloting referral pathways for crisis stabilization units and inpatient hospitals. Increase relationships and collaborations with diverse underserved populations including supporting development and implementation of a statewide process for incorporating culturally linguistically appropriate standards within Georgia's System of Care (treatments, supports, policies, evaluation, etc.) Develop and implement suicide prevention and implementation approaches throughout Region 3 including creating linkages with partners providing suicide prevention services. Collaborate and coordinate with State MATCH (Multi-Agency Treatment Team for Children) to facilitate collaboration across state agencies to explore resources and solutions for complex and unmet behavioral health treatment needs for youth and young adults and to provide for solutions. Monitor, track and evaluate youth, young adult and family interventions and outcomes through Carelogic and Substance Abuse Mental Health Services Administration (SAMHSA) online reporting system. Attend all mandatory Georgia Department of Behavioral and Developmental Disabilities (DBHDD)/ MATCH / System of Care sponsored trainings, orientations, and workshops addressing various topics and populations ~ including but not limited to youth, young adults, adults, substance use, cultural competency, and recovery. Minimum Qualifications: High School diploma/GED and three (3) years of job-related experience in a social services-related position; or One (1) year of experience at the lower-level Social Services Tech 2 (SST011) or equivalent position. Note: Must be at least 21 years of age and have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy. Preferred Qualifications: Bilingual English / Spanish speaker Knowledge of youth, young adult, and adult provider network in Georgia Department of Behavioral and Developmental Disabilities (DBHDD) Region 3 Knowledge of agencies serving underserved populations Experience with SAMHSA NOMS interview process and reporting mechanism. Experience with CareLogic reporting. Requirements/Competencies: Must be able to lift 20 pounds, Requires long periods of sitting/standing, Must be 21 years of age, Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
