Senior Primary Care Physician

Essential Job Duties/Responsibilities:

• Provide direct patient care and assessments in ambulatory health care centers and other settings
• Conduct geriatric assessments, medical history, physical exams, diagnosis, and treatment
• Participate in patient care rounds and conferences to communicate patient management strategies
• Collaborate with the multidisciplinary team to ensure successful patient care
• Advocate for patients and provide education to patients and families
• Manage resource utilization and reimbursement for services
• Facilitate discharge to appropriate level of care and use preferred providers for additional services

Education and Experience Criteria:

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
• Board Eligibility is required; Board Certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
• Current, active MD licensure in state of employment is required
• Minimum of 2 years' clinical experience in geriatric, adult, or family practice setting required
• Current DEA number for controlled substances required
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days of employment
