Palliative Care Nurse - Advanced Illness Management

Your Responsibilities:

• Conduct in-person and telephonic assessments for palliative care patients
• Provide patient education and support
• Coordinate care with the interdisciplinary team
• Document timely clinical records
• Facilitate care plan development and implementation

Your Qualifications:

• Registered Nurse with 2 years of prior experience
• Experience in palliative care, oncology, hospice, or geriatric care preferred
• Strong compassionate communication skills
• Competent in computer programs and electronic medical record documentation
• Current RN license and CPR certification
