A little about the job....
- Is accountable for coordinating care delivery for an identified caseload of patients in collaboration with medical staff and other members of the interdisplinary team.
- Facilitates pre-certification and payer authorization processes; facilitates collaborative management of patient care across continuum, intervening as necessary.
- Plans to meet patient needs to ensure high quality of patient care, reduced length of stay efficient utilization of resources and appropriate reimbursement of care and services.
- Apply process improvement methodologies in evaluating outcomes of care.
- Supports Clinical Documentation Management Program (CDMP) by performing medical record reviews, identifying correct DRG designation, ensuring documentation accurately reflects acuity.
- Serves as clinical resource for medical records coders and coordinates communications with physicians.
Qualifications for the job...
- Graduate from an accredited school of nursing and currently licensed by the State of Alabama.
- Bachelor's preferred (nursing or associated field) CCM preferred. Currently licensed as RN by State of Alabama.
- Three to four years' experience with one year recent clinical experience in specialized area or one year clinical case management experience.
- Utilization Review experience preferred.
