A little about the job....
- Serves as a support role to Case Mangers by organizing and arranging for services for discharge. This may include but not limited to planning treatments, financial assistance, living arrangements, social services, mental health placement, post-acute decisions, ordering DME, clerical duties and monthly QA and reports.
- Administration and application of the Watershed project (or like program) for the discharge process.
Qualifications for t he job...
- Bachelor’s degree or 7 to 10 year in related Healthcare field required.
- Must have knowledge of Healthcare environment; geriatric behavioral unit experience preferred.
- Must be proficient in general office equipment, Microsoft Office Suite (i.e. Word, Excel, etc…).
- Must have interpersonal skills to communicate with patients, family members, clinical staff/physicians and regulatory agencies.
