This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge. Qualifications Qualifications: * Licensed Vocational Nurse, per the state requirements. * Good communication skills to be able to relate professionally and positively to patients, caregivers, colleagues and physicians and strong clinical documentation skills. * Home Care Home Base experience preferred. * Two years' experience in Home Health preferred. * Ability to read, write, and speak the English language. Essential functions: * Under the direction of the RN, assist in identifying the patient's physical, psychosocial, and environmental needs. * Participate in planning and implementing care in conjunction with the RN, in accordance with the POC. * Provide care utilizing infection control measures that protect both staff and patient (OSHA). * Records treatments administered in medical record. * Records clinical documentation according to accepted regulatory, corporate and professional guidelines. * Participates in weekly case conferences. * Participates in interdisciplinary communication to ensure that patients' needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress. * Provides verbal or written patient status updates to supervising RN prior to reassessment, supervisory and/or discharge visits, and as indicated by patient response to treatment. * Participates in in-service training programs. * Responsible for assuring patient safety. * Complies with state regulations and national standards * All documentation will be completed and synched by 9:00AM the following day, 7 days per week.
