Salary Range : $120,000.00 per year with quarterly bonus opportunity

A Internal Medicine Group has an opportunity for a Full Time Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) to work with patients at home in the Wilton area. This is an excellent opportunity for those that enjoy a flexible work schedule, working autonomously, and accelerate in a pay for performance environment.
The Advanced Practice Provider will travel to patients' homes to provide primary care as their PCP. The APP is responsible for the delivery of patient care through the process of assessment, evaluation, diagnosing, recognition and prevention of adverse event, planning, and implementation. The APP will collaborate with the other professional disciplines to ensure safe, effective and efficient patient care delivery, and the achievement of desired patient outcomes.
Essential Function
  • Assess and examine patient in their place of residence
  • Function as the patients' PCP
  • Create a care plan for the patient
  • Utilize EHR Epic for all documentation
  • Engages patients and their families/support people in understanding and developing treatment plans
  • Analyze patients medical history, medication allergies, physical condition and examination to determine best care
  • Provide patient, family, and/or support people education
  • Diagnose and treat chronic and acute diseases
  • Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate
  • Orders and evaluates patient testing, including lab work and imaging.
  • Directs and coordinates the patient care activities of nursing and support staff as required.
  • Referrals when appropriate
  • Order home health care and hospice teams when appropriate
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards
Education and Experience
APRN
  • The Advanced Practice Registered Nurse is masters or doctorate prepared
  • Certified Nationally as a Nurse Practitioner through AANP or ANCC .
  • The Nurse Practitioner must have possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse
  • DEA and Ct controlled substance license
  • Experience working with geriatric patient population
  • 1-2 years' experience as APRN or PA
  • Experience working with EHR Epic
PA
  • Physician Assistant Degree
  • Physician Assistant with valid CT State license
  • DEA and Ct controlled substance license
  • Experience working with geriatric patient population
  • 1-2 years' experience as APRN or PA
  • Experience working with EHR Epic
