Essential Function
- Assess and examine patient in their place of residence
- Function as the patients' PCP
- Create a care plan for the patient
- Utilize EHR Epic for all documentation
- Engages patients and their families/support people in understanding and developing treatment plans
- Analyze patients medical history, medication allergies, physical condition and examination to determine best care
- Provide patient, family, and/or support people education
- Diagnose and treat chronic and acute diseases
- Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate
- Orders and evaluates patient testing, including lab work and imaging.
- Directs and coordinates the patient care activities of nursing and support staff as required.
- Referrals when appropriate
- Order home health care and hospice teams when appropriate
- Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards
APRN
- The Advanced Practice Registered Nurse is masters or doctorate prepared
- Certified Nationally as a Nurse Practitioner through AANP or ANCC .
- The Nurse Practitioner must have possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse
- DEA and Ct controlled substance license
- Experience working with geriatric patient population
- 1-2 years' experience as APRN or PA
- Experience working with EHR Epic
- Physician Assistant Degree
- Physician Assistant with valid CT State license
