Registered Nurse Case Manager
We are looking for a dedicated and experienced Registered Nurse Case Manager to join our team. As a RN Case Manager, you will be responsible for organizing, coordinating, and directing the ongoing provision of services to patients in their homes. This role involves making home visits in the community, collaborating with the patient's families and physicians, and maintaining accurate patient records. In addition, you will participate in the agency's Nurse on-call schedule, weekly case conference meetings, and the Quality Assurance and Performance Improvement (QAPI) Program. The ideal candidate will have a current RN license, a minimum of two years of nursing experience, and demonstrated skill in nursing practice and administrative performance. - Graduate of accredited school of nursing
- Bachelor's and/or Master's degree preferred
- Current RN license in good standing
