Home Care Admissions Nurse

Job Summary: The Home Care Admissions Nurse provides intermittent nursing service to patients in their homes under a plan of care established in collaboration with the patient's physician and according to agency policies and procedures. This role involves admitting clients to service, establishing a plan of care, and referring to other disciplines and agencies, as necessary.

Requirements:

• Graduate of an NLN-approved school of nursing
• Possession of a current license to practice as a Registered Nurse
• One (1) year of recent medical/surgical nursing experience
• Additional experience in community health nursing preferred

Responsibilities:

• Practicing and evaluating patient care based on professional nursing theory and practice
• Administering patient care according to Agency policies, regulations, and procedures
• Using medical equipment to provide patient care
• Establishing a safe work environment by identifying safety hazards and precautions
• Documenting in clinical records and using Electronic Medical Record
• Establishing and maintaining effective working relationships with patients, families, physicians, co-workers, and the community
