SHIFT DIFFERENTIAL FROM $0.50 TO $2.00 PER HOUR DEPENDING ON SHIFT!
POSITION SUMMARY:
This position provides medical care for residents to assure the highest degree of quality resident care is maintained at all times within a safe environment; assists with activities of daily living, offers emotional and social support, and attends residents overall physical comfort and well-being; also provides communication between medical providers, family members, and facility team for coordination of care.
ESSENTIAL FUNCTIONS:
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. To perform this job successfully, an individual must be able to perform each duty satisfactorily. Other duties may be required and assigned.
  • Assists and participates in development of total care plan of residents with physician, psychiatrist, nurses and allied therapists.
  • Participates in providing nursing care for residents.
  • Assists in rehab programs and bowel and bladder training programs.
  • Observes and reports on residents' physical and mental condition; obtains vital signs.
  • Keeps patient records and reports according to policy.
  • Provides leadership and directs functions of Certified Nursing Assistants.
  • Assists with admissions, discharges, transfers, appointment scheduling.
  • Notifies charge nurse of any change in patient condition.
  • Investigates and reports any allegations of abuse or care giver misconduct and ensures residents are treated with respect and dignity.
  • Administers medications and treatments as prescribed by physician and psychiatrist.
  • Contacts medical provider and family of residents related to change of condition or concerns.
  • Receives MD orders, makes rounds with MD's, transcribes orders, and communicates orders to nursing staff and pharmacy.
  • Assists in care and monitoring of IV's per facility policy.
  • Monitors and assists residents with psycho-social events.
  • Provides support, information or education to residents or family members as needed.
  • Other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES
  • Knowledge of the principles, standards, practices and techniques of Skilled nursing.
  • Knowledge of the dangers inherent in and the precautions to be taken in the use of drugs and other medication.
  • Knowledge of local and state rules, regulations, procedures and policies pertaining to nursing services.
  • Ability to assign and supervise the work of professional and nonprofessional personnel and to instruct them in practices and techniques.
  • Ability to accurately maintain patient medical records and prepare reports.
  • Knowledge of electronic records systems.
  • Knowledge of PPE and its proper use for personal safety measures.
  • Ability to maintain confidentiality of all resident care information and assure resident rights and privacy are protected at all times per HIPAA.
  • Ability to create and maintain an atmosphere of warmth and personal interest as well as a clean environment.
  • Ability to work tactfully and cooperatively with residents, families, visitors and entire staff throughout the organization.
  • Ability to verbally communicate with residents, families, the public and other members of the organization.
  • Must be able to exhibit a warm, cheerful caring manner.
  • Must be able to safely perform the essential job functions.
  • General knowledge of standard office procedures, practices, equipment and office assistance techniques.
  • Knowledge of County and department programs and policies.
  • Ability to prepare and maintain accurate and concise records and reports.
  • Ability to work the allocated hours of the position.
LANGUAGE SKILLS
  • Ability to communicate appropriately/professionally. Ability to communicate effectively with other members of the staff, supervisor, and the public.
  • Ability to communicate clearly and concisely in both written and verbal form.
  • Must be proficient in workplace English and spelling.
  • Ability to read County policies and procedures; written instructions, general correspondence; SDS sheets, safety manuals, maps, etc.
MATHEMATICAL SKILLS
  • Ability to calculate mathematical calculations.

  • Must be able to remain calm when called upon to assist residents during emergencies. Ability to understand and effectively carry out verbal and written instructions.

PHYSICAL REQUIREMENTS
  • This work requires the occasional exertion of up to 75 pounds of force; work regularly requires sitting, frequently requires standing, speaking or hearing, using hands to finger, handle or feel and repetitive motions and occasionally requires walking, stooping, kneeling, crouching or crawling and reaching with hands and arms. Work has standard vision requirements.
  • Vocal communication is required for expressing or exchanging ideas by means of the spoken word.
  • Hearing is required to perceive information at normal spoken word levels.
  • Work requires preparing and analyzing written or computer data and observing general surroundings and activities.
  • Work frequently requires exposure to bloodborne pathogens and may be required to wear specialized personal protective equipment.

  • Vaccinations and testing as required by local, state, or federal regulations and/or regulating entities.
WORK ENVIRONMENT

MINIMUM QUALIFICATIONS:
EDUCATION AND/OR EXPERIENCE REQUIREMENTS
  • High School Diploma/equivalent.
  • Completion of a practical nursing program.
  • Wisconsin Practical Nurse License.
  • Previous experience as a CNA or LPN preferred.
  • Must successfully pass criminal and caregiver background check.

Full Pay Range: $27.09 - $37.74
Expected Pay Range: $27.09 - $30.65
