Social Worker

DaVita seeks a Social Worker to join its team, providing support and advocacy for patients with End Stage Renal Disease.

As a vital member of each patient's core care team, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of dialysis.

You will build meaningful and long-term relationships with patients and their families in an intimate outpatient setting, and be part of a team that appreciates, supports and relies on each other in a positive environment.

• Building meaningful relationships with patients and their families
• Providing support and advocacy for patients with End Stage Renal Disease
• Collaborating with the care team to develop and implement patient care plans

Requirements include:

• Master's degree in Social Work (MSW) with a specialization in clinical practice
